Bristol last year, there was a crackdown on the pouring it into the provided vessel.  It seems that people were getting greedy, and there was a large enough percentage of "unpaid for" overage that all liquids were to be dispensed in the proper cup.  This was the case with the places I always frequented, and the counter staff was very apologetic about it.  Maybe not all the vendors did it.

I'm not sure if its always been this way but the past few years, generally at SRF if I asked to have my tankard filled instead of a cup at the bad dawg tavern they would. Not sure about the other vendors though as I usually pour it in my tankard myself.

At TRF and Scarby, they usually pour the drink into the proper cup, but will happily decant it into my mug and dispose of the modern plastic thing.

As for promotions, the place I bought my mug had a deal where if you bought a certain size (larger than I wanted), you'd get your first beer in it for free.
